# Sådan kan Fælles Stamkort bruges

Læs, hvordan Fælles Stamkort kan bruges af borgere og sundhedspersoner på tværs af praksissektor, regioner og kommuner.

På sundhed.dk kan borgere indtaste og opdatere deres stamoplysninger, fx telefonnummer, pårørende og sprog. Nogle stamoplysninger indhentes automatisk til stamkortet, fx CPR-nummer, folkeregisteradresse og sygesikringsgruppe.

Borgere kan give pårørende fuldmagt til at se og ændre stamoplysningerne. Det kræver, at borgeren giver den pårørende en fuldmagt via Digital Fuldmagt.

Hvis en borger har indtastet stamoplysninger, kan sundhedspersoner, der har borgeren i behandling, se stamoplysningerne i deres eget fagsystem eller via Sundhedsjournalen. Sundhedspersoner kan også hjælpe med at opdatere stamoplysningerne for borgere, de har i behandling.

## Hvem kan bruge Fælles Stamkort?

## Borgere

* Alle borgere med MitID
* Pårørende med fuldmagt

## Sundhedspersoner

* Sundhedspersoner med autorisation (fx læger, sygeplejersker)
* Sundhedspersoner uden autorisation (fx lægesekretærer, social- og sundhedsassistenter) kan tildeles adgang. Det besluttes lokalt, hvilke sundhedspersoner uden autorisation der skal have adgang til Fælles Stamkort.

Sundhedspersoner må kun tilgå Fælles Stamkort, hvis det er relevant for borgerens behandling.

## Hvad kan Fælles Stamkort bruges til?

## Det kan borgere

* Opdatere egne stamoplysninger, så oplysninger om fx pårørende, midlertidig adresse og foretrukne sprog er korrekte.
* Få hjælp fra pårørende til at opdatere stamoplysninger.

## Det kan sundhedspersoner

* Tilgå de rigtige stamoplysninger, når behovet opstår.
* Hjælpe borgere med at opdatere stamoplysninger.
